How does hypercalcemia result in renal failure?

Ok, I know in acute renal failure (ARF), the decrease in glomerular filtration results in a retention of phosphate (among other things...I know) and leakage of intracellular phosphate into the interstitial fluids. This hyperphosphatemia inhibits the 1-alpha-hydroxylation of 25-hydroxycholecalciferol to calcitriol. The resulting decreased plasma concentration of calcitriol causes skeletal resistance to the actions of parathyroid hormone causing hypocalcemia. Hypercalcemia in the oliguric phase of ARF is suggestive of a diagnosis of malignancy. So I can see how hyper-/hypocalcemia can result from ARF, but saying that hypercalcemia causes renal failure is a bit confusing to me. While studying up on multiple myeloma, I read that hypercalcemia can result in renal failure. The only answer I found as to how that could happen is that the hypercalcemia causes vasoconstriction in the glomerulus. Is there anyone that can shed some more light on exactly how this happens or if this is what happens at all?

I'm really confused, what is the difference between calcitrol and calcitriol ?

I see both spellings of it, sometimes in the same textbook diagram!

Alzheimer patient going into full coma, professional help please?

My aged father (90) has been suffering from Alzheimer’s disease for some years. He has had gradual deterioration of memory, and loss of judgement. Recently he has also been showing some signs of hallucinations. In the recent days he has proceeded into an almost complete state of coma. His eyes may be open, but he is not looking. He has been using Aricept, Propronolol, Citalopram, Calcitriol and other medications. Recently, he had a serious infection, due to which his left ear area was badly swollen, and his behaviour was obviously indicating that his brain was being effected by the disease. This infectious state was successfully treated using Ceftriaxone and serum therapy, and he almost returned to his previous state. But, we suspect that the infectious disease effected his brain in some way. We are also suspecting that he may have undergone some trivial brain infarct. His life functions indicators (heart pulse rate, respiration, temperature, blood pressure etc.) are almost normal. He is able to move almost all movable body organs. Blinking function, swallowing etc. are intact. The doctor has instructed discontinuation of Aricept, and has additionally administered Nootropil(Piracetam) and Osvix (Clopidogrel), but he is advancing into full unconsciousness. Will appreciate your professional advice as to the real state of affairs, and how it my be encountered.

My Shih-tzu was diagnosed with early kidney failure and has had diarrhea for 4 days?

My 9 lb. Shih-tzu (almost 14) was diagnosed last Monday with early kidney failure. My vet immediately (Tuesday) gave her fluids under her skin and put her on Calcitriol and Purina NF (which she loved). Then on Wed. night she started with diarrhea and wouldn't eat or drink anything. I took her back to the vet Fri. morning and he could not find a reason for her having diarrhea. He gave her a Centrine injection and VBG 1 ml and put her on 1/2 Centrine 0.2 mg tab every 12 hours. Yesterday the vet switched her food to another for kidney failure (I don't remember the name of it) but she doesn't want it either. The only thing she seems to want is plain turkey or cheese (to get her to take the pill). I also tried the food she used to eat (Iams mini-chunks) but she won't eat it. She is drinking a little water. She has diarrhea every 1 1/2-2 hours and is a very little amount sometimes with a little blood. Any ideas on what's wrong and what I can feed her? Please help! please help me, this question was for my mom, she is taking fosamax or ALENDRONATE every saturday and a calcium called calcitriol, sorry for the names but they are in spanish, we are mexicans and we live in mexico, but I didn't get answers in spanish about these medications so I am asking in this section, well I am learning english sorry if you couldn't understand my english but it's urgent, because a doctor told my aunt that fosamax is the same that taking calcium but my mom takes fosamax the simple not the new version with calcium, but please help me is the same thing for helping osteoporosis thank for your help and explain me the work of these medications in the body. btw my mom has like 1 year taking both medications for it.. greetings from mexico to everybody bye.
